A realistic fiction about an upper elementary class of students who have low test scores. They spend the school year reading real books and journaling about them to each other and the teacher. At the end of the year their scores are way up. I think they also save their teacher's job because of the higher scores.I read it probably about five years ago, don't know when it was published.

My first thought was the same as Kris's suggestion: The Freedom Writers Diary by Erin Gruwell.

The description also reminds me of "My Posse Don't Do Homework" by LouAnne Johnson.
