It is hardcover. That appears to be it, but my copy has 216 pages. I have scanned it, but not sure how to get it to you. (Sorry for being dense.)

Most people add the scan to the photos on their profile ('add more photos') and then we grab it from there, but I don't know if that works with a profile set to private.
