I've got some more information on this release as well as a cover scan.

This book seems to be the first edition; it has "Copyright 1936" on the page before the dedication. The "master" book (https://www.goodreads.com/book/show/6...) says that it was first published in 1964, but there's a bunch of variations within it (including this one) which were published before that date.
