I'll explain this for Klownzer.
There are psychologically multiple people who control Klownzer's body, this usually happens due to repetitive early childhood and late childhood trauma as a developed way for the brain to cope and survive.
About 6% of patients with Dissociative Identity Disorder (Used to be known as Multiple Personality Disorder), are openly switchable. Most times alters (that's what you call the different people who take over the brain) mask as the host to protect the body and the mind itself usually hides what's going on with the brain from the host (The original / primary alter who takes control) to protect them.
Different things like Caretaker Alters (Personalities / Identities which take care of the host, My self [Toshinori] is an undiagnosed example.) typically act as nurses, doctors, parents or figures in the hosts life because of what they didn't have as a child. In some cases (somebody who's more educated correct me if this is wrong) the caretaker will put the host to sleep or even take over completely as a means to protect them. Like a guardian lock.
Another type of alter known as a persecutor attacks the host because it has the believe that that will end their suffering (sometimes its different). Persecutors can cause the host to relive suffering over and over.
Alters can be anything under the sun (Alien, Human, Fictive [based off a character], Factive [based off a real person], Animal, Child, Adult, etc.) and usually develop between the ages of seven (7) and nine (9), rarely later which means that the body is dealing with instability of identity for years before a true diagnosis can be made. There can be hundreds upon hundreds of alters inside a body which can all have different names and memories as well as different ways of presenting when at front (aka in control).
Alters can speak by themselves and talk to each other depending on the way the brain develops. People with D.I.D. could hear voices and see things as well. The brain itself is fractured into pieces.
Usually only people over eighteen (18) have said diagnosis be on the board as official. Schizophrenia is under 0.1% possible in people under 25 which means that doctors will check for things like D.I.D. and Borderline Personality Disorder as a prefab. Anything diagnosed before eighteen in that area is known as ``early-onset``.
People with said disorder are least likely to seek help due to their brain trying to get them to believe they are just tripping (delusional, self-sabotaging) and not sick. This means the mind will try and find reasons for the disorder not to be real to cope with its coping mechanism. Individuals can experience loss of memory on the daily and even may end up dissociating at random periods to cope with situations even if its not traumatizing because the brain is wired to trigger it.
Dissociating is different for everybody. For me particularly, I have a hard time seeing even if I'm not actually blind. My body stops functioning, things like immediate lack of motivation and sometimes control. The world seems distant and the color starts to drain, objects may split into three like a fringe and I may see colors that don't exist or things that don't either. Imagine falling asleep with your eyes open. The body may start to feel disconnected and not attached to the person. They may see imagery that is unrelated to what is going on. They may go deaf.

I'm aware of what you were saying that to. Context not required. I replied to something random you said so you knew I was speaking to you.
I offered to explain because the business-like speech can sometimes be confusing. I was humanizing and simplifying information so you didn't have to go on a trip to timbuktu.
