Probably my biggest pet peeve when it comes to books is when they change the covers mid-way through a series. Either this or when authors purposely shorten or extend their series so that it becomes a trilogy because it's trilogies are popular at the moment. Usually when that happens you either have a filler book that doesn't add anything to the plot or a very rushed third book where the ending is ambiguous.

That is my biggest pet peeve as well! Especially if you are buying each book as it comes out...

I agree with Bridget too... irritating. I also hate it when they incorporate erotic scenes abruptly in the story and you get WTH feel about it.
When they decide to turn a book into a movie. The movies are all terrible and can never measure up to the greatness of the books!!
I hate when book one is great in the series and I get attached to the characters but book 2 is a companion novel from a different POV.
... 16 year olds act like they're 10 and authors try to be cool and modern but fail, that's the worst!
When the MC girl gets so obsessed with the guy she likes that the LITERAL END OF THE WORLD or collapse of society or whatever no longer matters because OH MY GOSH HE'S SO HOT I THINK I MIGHT BE IN LOVE even though she hardly knows him.

It also annoys me when an author has a character talk to themselves and put the thought in italics, followed by "s/he thought."
