Who has a reading journal?

I love working on my reading journal at the beginning of each month and was curious who all has one and what formats people use. I tend to have 1 book I got off of Amazon that just keeps track of each book I read/rating. Then I have a journal (this year im using a coraline one) and actually make like a monthly spread, tbr, favorite book of the month. So relaxing

I've always wanted to have one but I am very disorganized, I just vaguely write my thoughts here on goodreads and sometimes when I have a lot to say of a book I do use a notebook but it's not only for books (that notebook has everything you can imagine inside, from a cooking recipe to my existential crisis haha)

I have one where I write down every book I've read throughout the year. It has some questions like if it's an ebook, audiobook, pocket or hardcover. I can also make small notes about what the books about, what I think was good/bad about it. Sometimes I'll go back and check if the scores change if I've reread a book. I think it's very handy to keep track of books so when I'm investing in new books (I'm a library girl) I know which ones I liked and want to buy for my own shelf.
