This is information I received from the Author, Is there another way to confirm this was just a placeholder and never used?

We ask the author if the cover image was ever distributed, such as with an ARC or review copy.

If so, we would mark the edition as an ARC with a Librarian Note and "Advance Readers Copy" in the Edition field.

If not, we would merge the edition.
