Was first tempted to ask for this book to be combined into the main entry, but I'm guessing that "Not Avail" as publisher stands for "not available", plus wrong author (listing both as one), unknown binding and no cover, may be a bad entry that should be fixed or even removed?

Fixed author and combined.If you can let us know what the other data should be, we can correct those as well.
Not sure, just happened to stumble upon that entry.Only actual result I get on a quick search for this ISBN with a similar number of pages (592 instead of 593) is https://discover.libraryhub.jisc.ac.u...
(There's also https://discover.libraryhub.jisc.ac.u... but that lists 578 pages).
From that, I see publisher as Guilford Press, as all other editions, and apparently published in 2007.
About fixing the author though, there are two of them, Phillip R. Shaver being the second.
