Have you read Sybil by Flora Rheta Schriber? It's not a fiction book so there is no revelation or twist but it's a really fascinating read.

Would The Basic Eight, by Daniel Handler count?

(Not exactly a positive portrayal, but an engaging narrator).

I'm Eve by Chris Costner Sizemore, the woman upon whom The Three Faces Of Eve was based. The Bird's Nest by Shirley Jackson is probably not altogether psychologically accurate, but it's well-written, as you'd expect. There's a hilarious scene in which the girl's personalities take bath after bath, in the process using up all of her aunt's new bath salts.
