I have made several requests to join the author program and keep getting a rejection notice. I have supplied all the correct information and linked it to my book listed on Amazon.

You should reach out to Support directly. Librarians are volunteers and have nothing to do with the author approvals.
I have, however, changed the format of the book to Kindle from ebook and merged the edition that was created by mistake. It will go away in a day or so.
The Adding New Books folder is for requesting new books to be added. Next time use the Book Issues folder.

Thanks, Emily for tidying up the formats. That could have been the issue with the author's program acceptance. I will try again the next day.
