They have an affair and the h thinks it forever, but the H tells her he has to go back to his country.
The h is pregnant, and the H doesn't know it.
The baby is now a toddler, maybe older(can't remember) and needs a expensive surgery to live. The travels to the H's country, and the H thinks she's there to disrupt his planned marriage to a Princess from another country. So he has her locked up until he finds out there really is a child, and that child is incredibly ill.
