This is my scan of my personal copy.

The entry for this book also does not list page numbers, but the book itself does not number them, so I believe protocol is not to include page numbers?

I found a cover image on the author's website, so I added that. I also added the Kindle and hardcover editions, in case anyone is interested in those.

It gets confusing when a thread linked to a specific book or author is also used for other books. I am closing this thread. Start new ones for other books.
