When they are back in the US she goes to the clinic he works for. Theres a nurse who is jealous of the FMC and gives her a cream and some tea, this leads to the FMC going into early labour and being rushed to hospital, she gives birth to twins but has complications she has kidney problems etc as a resullt.

I've read the book a number of times but I've also read the same authors (80+books 2-3 times)
