Appears to be errors with ratings on a book (duplicates, etc)

This isn't really a librarian issue, I don't think, but the reason multiple ratings appear is because these users have rated multiple editions of the same book, which is in accordance with GR's design. If you wish to confirm this, you can click on the dates under the ratings to see their full review.
