That's when I suspected it.

The author placed emphasis on Steve drinking that orange juice from the carton so I became suspicious then. When Kyle got sick, that sealed the deal.
The thought was in the back of my mind when Kyle drank from the OJ container and got dizzy. When Patrick got sick and Lucy traced it back to his water, I was pretty sure. I didn't think anything of Steve drinking from the OJ container. Lucy did a lot of reminiscing and I just chalked it up to that.
I also noticed the emphasis that the author put on Steve drinking from the carton of orange juice. As soon as Kyle fainted and we learned he had been drinking orange juice also, I figured that was why Steve kept getting sick.
