I say its usually the radio man. He's got just one job to do, but somehow his radio always fails just when its really needed.

I don't agree, for the simple reason that, in a small SF team on mission, the team commander is supposed to control and supervise his men, thus is ultimately responsible for the success or failure of the mission. If he had a dud as part of his team, then he should have identified him before the start of the mission and replaced him. This sounds harsh, but it's the reality of combat command.
