What is the rule when the real name behind a pseudonym is known ? Should it be included as second author ?

No, only if the (Goodreads) author requests it, or if some editions were published under the real name (cf. Barbara Vine, whose books were published under her real name Ruth Rendell in some countries).

Some books which were originally published under a pseudonym, may have been published later with the author's real name or even, in fact, with another/different pseudonym.

Only if some editions are published under the real name, they need to have both the pseudonym (as primary author) and the real name as secondary (or vice versa, if the book was first published under the real name).
