If you go to your 'my books' section on goodreads, (making sure you have it on list rather than covers), on the far right you'll see a little link next to each book which says 'view'



If you click 'view' next to the book you are after, it will bring up your review for you, then all you need to do is cut and paste the URL from the address bar at the top of the screen -



and that is the link to your review ☺
