SOLVED. Book starting with newborns (twins?) being baptised by sister in river. [s]

One of the first things in the book is the sister bringing these two new babies to the river to baptise them as her parent didn't want them to be baptised but she kept being scared of them going to the limbo. She is discovered doing so and [spoiler ahead] accidentally lets go of one of the babies who drowns before she could complete the baptism. I don't remember anything else

But I can't remember the reason that the sister was in the river with the babies in that book.

Rainbow heart unfortunately I don't know, I was a little kid and I read it over my father's shoulder on a plane over fifteen years ago. It was just traumatising so it stuck

Was it in English or Italian?

If your dad was reading it, it sounds like it must have been for adults.

Probably in Italian since he was reading it, though it might have been a translation because names were all in English. Anyway I knew it was a super long shot so it's fine, thanks for trying to help ❤️🥰
