ABANDONED. Short Story About a Woman dying and returning to the land where her ancestors are

If I remember correctly, this short story was on Facebook when I came across it. It's a short story and it was so beautifully written. From what I remember, it's about a woman who was lying on her deathbed in a hospital, knowing that she was going to pass (I can't remember if she was dying from an illness or from an accident). She remembers hearing her husband and children's cries as she finally takes her last breath. Then, as she takes her last breath, her spirit goes into the afterlife and she crosses rivers and climbs mountains. Finally, she arrives on a land where her ancestors are at and they welcome her in open arms. That's pretty much all I can remember how the story went.

I'm not familiar with stories on Facebook. Is this something that was actually published?

Honestly, I do not recall. I think it was a published short story, a short story you'd find in those elementary schools. I just remember coming across it in Facebook.
