While searching for the ISBN, I found this:http://ipac.lib.bbk.ac.uk/hipres/info...
The author's name there is Allen Thompson and 2 ISBN are listed:
1557867232 (computer disk)
1557867224 (manual)

Strange that the replacement disk is listed under a different name on almost every sites.
As for the NAB question, I'd tend to say that an accompanying disk without its book can't be counted as a book.
But I'm a rookie here, maybe a seasoned librarian would have another opinion.
Sorry I've more added to the confusion than solved anything.
