Working out the ending--Ways to Hide in Winter

Ways to Hide in Winter
Note: This discussion thread is obviously for those who have finished the book. **Spoiler alert!**
To start, I really enjoyed this book. I give it 5 stars. I believe it is a book that should be discussed, hence this thread!
The one thing I can't figure out about this book is the ending. What exactly is the significance, and how does it shed new light on the main character, Kathleen? What is the big twist?
The novel ends with a brief mention of two historical events. What is the significance of these events?
Thoughts, anyone?

Everyone told Kathleen that she was too smart to stay in that small town- that she should go and make something of herself. At the end, she finally did. The part that seemed a little odd to me was that she left while her grandmother was dying instead of waiting until she passed. But whatever her grandmother whispered to her in the hospital must have made her feel like that was what she should do.
