Your description is exactly what I have in my head! For the longest time, for some crazy reason, I kept picturing the study on the 2ond level....

It always seems like everyone gathers in the space between surgery and the study. I guess it sort of like an entryway or foyer that leads into the living room and kitchen. For some reason in my head the surgery is on the left and the study is on the right. I don't know why, I don't think DG was ever that specific about the placement of the rooms.

The kitchen/living room was hard for me to picture, because I had all sorts of furniture way too close to the fire and in the way of the cooking. This helped. I found this picture of a colonial era kitchen online, it's from the Governor's palace in Williamsburg, so we have to assume everything is fancier, but a good idea:
