Possible spoiler... like I said, they met at BYU, but since he wasn’t a member, they couldn’t be together. So IIRC, they went their own ways only later to reunite. The celiac diseas was a one off character.

I’d say time was the 90’s. I do t remember the cover. Plot- she doesn’t marry him because he’s not Mormon. But her life doesn’t turn out so grand. They meet up years later. I think they end up getting together.

I did a quick search and the book that came up (no idea if it's the one you're looking for) is First Love and Forever by Anita Stansfield. It's part of a series (three books with the same two main characters but I think there are other books as well).
I think it was published in 1994.

They were college sweethearts at BYU. While in college he proposed marriage, but she decided to marry someone else because Michael was not LDS. 10 years later, their paths cross again and this book is just the beginning of their story.
