By reading your comment, it seemed to me that the Clones were a combination of the two principles: they are mades to work, as a Carer first then as a Donor. The work ultimately kills them, like overwork killed lots of slaves.
From Miss Lucy, we learn that places like Hailsham are not common. If compared to animal right, it migth be an attempt at free-range farming. In a sense it is a sort of deadlock: if you don't treat them as human, Hailsham just lose money so it is closed down ultimately. Plus, people might not want the organs, wich is, thinking about it, in itself terrible. If you treat them as humans, Hailsham could have become a standard, and in that case all other places that don't meet the standard close down.
Maybe, in that version of the world science is, in a way, the new religion. The Gallery serves as the medium between believers and preachers. It came as a miracle for the world after the war, solving the organ donation crisis. It can cause fear, like we saw with the incident leading the the closure of Hailsham.
Regarding the ratings, I generally rate according to enjoyment as well. Even though I am used to lighter themed book, I had as much enjoyment thinking about all those questions asked by the book. If the book, bleeds that much outside of the reading sessions, it cannot be bad in my opinion. Plus, I also really like the writing style. (hide spoiler)]
