I'm looking for a book that I read back in 2012/2013 about a girl who is concerned that she could unknowingly be a serial killer. The novel is, I believe, a standalone, and the protagonist has an older, highly successful brother and still-living parents. The protagnoist has faced psychiatric issues throughout her life, and now in her early twenties(?) is trying to function in broader society. She is possibly trying to attend college and has a guy who is either a friend or a roommate who is regularly in her life. The guy has something odd about his face, possibly a lazy eye, and the two become half-hearted love interests.

As the protag is trying to function, she stumbles across/wakes up in an (abandoned?) house with a corpse in it. She can't remember killing the person, but she doesn't remember "not" killing them either. Then other people die. The authorities suspect that she knows something, but more importantly, the protagonist is convinced that she has done something terrible and simply can't remember it.

And here is where the spoilers come in.

The protagonist's older brother has always been the golden child of the family. In some ways, she felt like it was the two of them against the world, but she also resented him for having such an easy time of life. It turns out the brother is a sociopath who has been gaslighting the protagonist her entire life. He killed people to keep her psychiatric issues alive and well and to ensure that she have to continue to rely on him. (...There are som weird undertones that accompany all of this.) When the protagonist discovers this, the brother reacts badly. As a last effort to retain control, he slights his wrists in a bathtub and calls the protagonist. He doesn't intend to die (I think); he just wants to frighten her. The brother miscalculates though and bleeds out in front of the protagonist while she is on the phone with someone (her roommate? 911? The police?)

The book is an odd one, but it is driving me crazy that I can remember all of these plot points but not what the book is called. Does anyone have any idea what the book might be? I'll be eternally grateful for any help!

Is it YA or adult? Where is it set, country/region/city etc.?

I'm not entirely sure. It was definitely set in the US. Possibly a coastal state. As for YA vs. Adult, it could have been in either. I think it was a used book that I picked up somewhere, and it didn't have super clear labeling on it.
