And @matthew, that's a huge spoiler.. Why are people commenting on what happens in books 2 and up? Remember this is a thread about book 1. Walder Frey, for sure. Say what you will about Petyr, Theon, et al. But that montrous coot betrayed and murdered Robb, Catelyn, and countless Stark bannermen for no reason except anger and petty retirbution. Still waiting for him to get his comeuppance!
Theon Greyjoy. He was raised with the Stark kids and was, by Robb at least, considered a brother. And when he betrayed the Stark cause it was a mortal wound because Robb Stark literally trusted him with his life. He stood up for him when His mother berated Theon's character merely for being a Greyjoy. Ultimately Theon proved her right and it was this cold hearted, not fully thought out betrayal that would send Robb on a downward spiral that would cause him to lose the war and his life.
Mirri Maz Duur. Daenerys saves her life and takes the woman into her service, the woman thanks her by killing her husband and unborn child and cursing her.

Cersei was the first one who came up simply because of her personality.
No offence intended here but I think since we are in a discussion forum about the first book of the saga, people should limit their answers only to that book. Some people are mentioning things that happen far ahead in the books and I for one would like to roam around this forum without being afraid about reading spoilers.
@camille does that happen in book 1? No then why is it here in thread about book1??? Thanks for ruining it for me..

Remember this is a thread about book 1 so you shouldn't give away what happens in the later books that people may not have read yet - I'm actually only 30% through the 2nd one. Jaime was the first who came to mind. Don't get me wrong, I'm not saying he shouldn't have killed Aerys. Only a fool (Ned Stark) would have followed the Mad King to the grave, but he did betray him.
And Lysa, don't even get me started, and Cersei if she really was involved in Robert's death.

It's strange, my very first thought was Shae although I know that her betrayal (if you could even call it that, after all she was being paid) was far less pivotal or consequential than the betrayals of characters like Theon and Baelish.
There were several betrayals all along, but the first name to pop up in my head is Littlefinger!
Betrayal implies there was trust to begin with. So - I have to agree with Theon's betrayal of the Starks.
