The girl attempts suicide after sisters death. Guy meets her on top of the hospital roof and yells at her. He is the bf of her sister who just died. Thinks he lost his unborn child too. They exchange numbers and say they will contact each other if they ever need the other one and they have to show up. Book goes through these encounters and they end up dating at one point but then break up. There are flashbacks mixed in with present day. In the present day the main girl is trying to foster her dead sister's daughter who never actually died. The guy doesn't know any of this until the very end.

I actually just read the book earlier this year but can't for the life of me remember the author or title. All of the following info is a spoiler.... The sister had cystic fibrosis if I remember correctly. She died of complications during pregnancy. They didn't know they were sisters at first. The one girl was in foster care and her sister got adopted I believe. The daughter also had the same illness. The two sisters were not twins.
