See problem with subtitle for the hardback.

Softcover needs a cover image - cover of softcover is same design as the hardback.

Corrected the subtitle for the hardcover according to the cover.
Added the cover for the paperback from Amazon (we can't just take the cover from another edition).
Combined the different editions.
