I like the drama. Either one or both of the couple are cheating on each other and know about it.

Sorry I don't know the book you are talking about - but the Regency Romance Lady Delafont's Dilemma dilemma comes to mind. Lord and Lady Delafont have been estranged for years but then are reunited when someone tries to kill the husband. (As I recall).
