Help Correcting a Series

This is actually a two part request.

First, this book isn't showing in the series correctly, and when the author (a client of mine) tries to correct it, it boots her to the home page rather than letting her add the series (under Edit Book Details > Work Settings). I tried following the same step on a book of my own, and got the same result. Can you guys fix this for us?

The second request is to merge the duplicate listings. She's got two listings for Delirium right now:
