Getting my book listed under the correct author

Hi! I'm trying to get my book listed under my profile. It's currently listed under another author. I'm also waiting to hear back from the Goodreads Author Program which might correct some of these problems once my application has been reviewed. My book is currently listed twice under an author of the same name, one with a cover and one without.

If I'm not doing this correctly, please let me know.
