Author name-form questions

Two questions about specific authors, with general implications.

Firstly:
The Science of Life, or How to Live and What to Live For was written by "A Physician". This appears to be the only name attributed to this book. Should it be credited here as "Anonymous", "Unknown", "A Physician", or just "Physician"?

Secondly:
Miss Honey Pie writes "fictionalized account[s] of real sex between real people complete with their own opinions", some of which include the subjects credited as "Narrator", should these be left and corrected as Interviewee, or removed (and possibly added as characters). Some are credited as initials (E.g. M, C) and others are forename only ("Zoe").

For the first one, I think it should be "A Physician".

For the second, those definitely don't seem to be authors. Adding them as characters is an interesting thought, but maybe not necessary.
