I've been meaning to ask this for quite some time now:
When adding a cover from a non-Amazon source (i.e. a library- or publisher-site), do I need to ask permission from the book's copyright holder?

Publisher and author sites should be fine: they want to have the cover out there, and they actually own the rights. (Exception: any site whose terms of use put limitations.)

Library sites vary. Some are ok, but some are not. Check out their terms of use if you are not sure.
