Is number four an unreliable narrator? Do some things he says seem hard to believe because he is mentally unstable?

yes. He isn't THAT mentally unstable but at parts he may panic or whatever and that's when he becomes a unreliable narrator.
Many times he does become out of control and that can make him an unreliable narrator. He can be crazy and make things up that he thinks he saw. Any one else have a though?
Four becomes psychotic sometimes because he wants to be original kid but he can´t. He needs to protect his kind in order to stay alive. He becomes crazy and that is when he becomes a unreliable narrator.
