I usually draw it to plan it out and then paint over it.

On the left you can add like a small picture (teddy bear with a balloon; the number sixteen, fourteen, or fifteen depending on which big birthday; cake; etc..). Where the writing is perhaps add like a twirl of vine? I dunno how to explain that, but I have a feeling that I made no sense...
