Add new paperback edition, then combine

I hope I am in the correct place:

I've tried several times to add a new edition (paperback) of my book to the existing Kindle version listed. But after I fill out the form I get an error message stating that the author name needs to be filled in (which it is), along with the captcha code (which doesn't appear at all).

Is it possible to get help adding the paperback version of Wild Suspicions and then combining it with the Kindle version?

Cover, description, author, bio is the same for both. Other info follows:
