how realistic is this book?

The abuse supposedly started at the start of the marriage, but surely when the two children were born, someone would have seen Kate's scars?

the physical abuse only started after the children were a few years old, "when Dominic and Lydia had been small" - it's in the description of the first holiday in Padstow, when Mark makes the children chose between him and Kate and insults her in front of them. When they are asleep she asks him "not to turn the children against" her. "It was the first time that he had properly struck her." He twists her words into "you don't care what I do to you" and proceeds "to punish her". (I've read this as an e-book and the episode is described at location 1180, 24% of the books, in the chapter "5 years ago".)
