The first author listed is Mary Graham, but I seem to be unable to add her as an author. I know she does not currently have a Goodreads account.

The reason for that is that it is too close to Margaret Graham. There are some Librarians who know a way to force it, but I am not one of them. Hopefully one of them will wander by this request.
Done.
Also reverted ISBN change on the related book. If there is a new edition, it needs to be added as a new edition, NOT have an existing one changed into a new one. And Smashwords is a publisher, not an edition type.

Thanks. I did sort of wonder about that, since I was also unable to add myself. (M.R. Graham - I also write as Margaret, and wanted to keep them separate but connected, if that makes sense.)
The previous ISBN is registered, but not published. The information attached to it is not attached to a book anyone might own. If it can't be changed, that edition should probably be deleted.

Looks like it was published (On Sept. 18, as Google's cache of that Smashwords page confirms), but has now been removed from the publisher's site. Goodreads does not delete out-of-print books. Please see http://www.goodreads.com/help/show/22... and http://www.goodreads.com/help/show/32...

Sorry, I just thought that if the issue was users being able to add the edition they own, and no one owns that edition (No one does. As the manager of that account, I can vouch for that), it's just clutter. As for Smashwords, would that information go in the 'publisher' field? Smashwords insists that all their publications be labeled "Smashwords edition," which might be what threw me off.
