ABANDONED. Forest paths lead to other worlds, surgery extends lifespan...

Read a book around 2005 (and I think was published around that time) of which only a few distinct details stuck yet I have this urge to find it and reread. I recall a futuristic human world where people get a surgery to keep them young when they start aging and a forest with paths that lead to other worlds. I know that's not going off much, but thanks if you have any leads.

This is an older book, but could it possibly be An Alien Heat (or something else in that series)? There are forests; the paths don't lead to other worlds exactly, but the people of the future can change their environments however they want, and also their bodies.
