ABANDONED. Sci-fi book about a brother and a sister(who is missing their lungs)

Read this book back in 2008 about a brother and a sister in a sci-fi/fantasy setting where strange events would happen and people would be physically changed. There was a description of an old man being turned into a little girl. The sister loses her lungs because of this and has to wear a rebreather. I can't remember what was wrong with the brother.
