How Can I Be Added As A Contributor?

I wrote one of the stories in this book (my name is even listed in the description). Is there a way to be added as a contributor, so it will be listed on my author profile page?

(I also noticed that there are three entries for the book on Goodreads, at least one of which looked blank (no cover art, not much info, etc)).
