I am a new author on Goodreads. I have not yet used anybody else, but I have stumbled across a company called New Shelves. http://www.newshelves.com/ - I have no information about how they do. I plan to contact them in the near future. They do use Lightning Source for printing but then claim to expand distribution to more difficult to reach markets.
In addition, if you have proven sales, then approaching Baker & Taylor directly for the library market is also an option. I am just beginning and these are aspirational goals - someday to be able to prove sales and crack open the Baker and Taylor door. http://www.btol.com/suppliers_supplie...

They printed great hardcover and paperback editions or me, but they are not a distributor. Good luck! - Mm

edit, nevermind, link seems to be good, just looks like their website is down.

Quality has been good for my books but they are not particularly cheep. I would have used Createspace if they made it easier for non-US self pubs to operate.
