author with multiple roles

I'm adding a book which is an anthology. The illustrator also has a story included. What is the appropriate way to add the roles? Should the name be entered twice, once as illustrator and once as contributor? Or should I say "Illustrator & Contributor" in the box?

If you add the name twice there will always be the possibility that it will be seen as a duplicate (either by a librarian or by one of the dozy bots/scripts) and deleted. I would go with "Illustrator & Contributor".

Actually, we more often use a slash than an ampersand, so contributor/illustrator.
