A possible long shot: Logan's Run (http://www.goodreads.com/book/show/73...) depicts a society run by teenagers in which anyone over 21 is killed, initially because of overpopulation concerns. The main character has just reached the age limit and decides to flee the system that he has served all his life. I don't remember Canada or medical vans though (since everyone just has an implant that measures their age). Like everyone else on this thread I hope you find this book since it sounds interesting and at least similar to Logan's Run, which was a great read.
Thanks for the post Jillian. I don't think the book was Logan's Run since I don't remember anything about a flower changing color. Looks like Logan's Run is worth a read though!
I don't know if this matches but it is the book that the move Soylent Green was based.

What about Sins of the Fathers by Will Cunningham?
