Would like to add my credits to a book (as well as a few other things)

Hello. My name is Mike Boldt and I would love to add my credits as the illustrator to this books profile. I am guessing this was done when it was a work-in-progress as the name of the book is actually "Little Jack Horner Live from the Corner" now.

We cannot use bookseller data (which Indigo, and it's subsidiaries most certainly are), but I got the image from a governmental archive site. Also I corrected the title.
