I'm not much of a fantasy reader myself, and I didn't give this book a particularly high rating, but Fireflies by Ally Blue might be something you're interested in (if you're cool with sex scenes, which it has). Other reviewers seemed to have enjoyed it more than me so it may be worth exploring. Also, I'm not totally sure if this is fantasy or just a more medieval setting but I know some people who adore Captive Prince (I think it's a series). I haven't read it but there's a pretty big fan-base on Tumblr so I see stuff on my dash pretty often. Sorry I can't be more help, but I'll let you know if I can think of anything else.
i've read and enjoyed the first two captive prince books, i hadn't heard of fireflies though so thank you for that rec! :D
I'd recommend anything by Poppy Z. Brite. Now granted, her writing is less fantasy than horror/thriller ("Lost Souls", her first book, is a vampire novel), but she writes pretty much exclusively about gay, transgendered, and otherwise non-straight/mainstream men.
She always identified as a gay male in a female body and has (I'm weak on details) undergone some degree of gender shifting, since writing most of her books. He now goes by the name of Billy Martin.
I truly loved "Lost Souls" (which has a gay male protagonist by the name of "Nothing") and its kind-of-sequel.
