I don't have a book recommendation, but I do recommend Khan Academy. It's free. Sal Khan originally started making the videos to help his nephew with math, and it became something much larger. There are videos and practice modules for math starting at the most basic level of counting and going all the up through the more challenging ones such as differential calculus and linear algebra. There are also other topics available as well.
