In the US and in Canada, the gender identity clinic – a centralized institution that holds a monopoly on access to care – slowly gave way to a more flexible model, in which multiple health centres could initiate hormone treatment more quickly on the basis of the patient’s informed consent, without any need for a formal process of diagnosis.
