If I take a treatment and then feel better, that seems like powerful evidence that the treatment worked. You cannot draw this conclusion, however. Perhaps I would have felt better anyway. Maybe I even would have felt better faster without the treatment. The only way to know is to look at many people taking the treatment versus a fake treatment (placebo) under controlled and blinded conditions and then compare all the outcomes.
